The Business Office is located on the lower level of the Appleton YMCA at 218 E Lawrence St, Appleton WI 54911.
Visit the SWAP drive and click on the 02 Standard Operating Procedures (SOP) folder, then the sub-folder you need. All current SOPs are located there.
A Certificate of Insurance is issued by our insurance carrier to a specific holder and confirms the YMCA of the Fox Cities is insured. If insurance is canceled, the certificate holder is notified. To request a certificate, email gro.seiticxofacmy@elbayapa with the name and address of the requesting party. Please allow three days of lead time.
