Reclaim Your Health
Whether you’re facing a chronic disease diagnosis, recovering from surgery, or managing a nagging injury, the Y is here to support you. Our Disease Management programs are designed to meet you where you are — providing compassionate, evidence-based support that helps you rebuild strength, restore confidence, and improve your quality of life. Trained Y staff lead each program with expertise and care, working alongside your healthcare providers to support your journey toward better health.

Our Programs
We offer three specialized programs for individuals managing chronic conditions, recovering from surgery, or living with serious health challenges.
LIVESTRONG at the YMCA
Cancer is a life-changing disease that takes a tremendous physical and emotional toll. The Y and the LIVESTRONG® Foundation have joined together to create LIVESTRONG at the YMCA, a research-based physical activity and well-being program designed to help adult cancer survivors achieve their holistic health goals.
Pedaling for Parkinson's
Group cycling and support helps reduce symptoms in individuals with Parkinson’s disease and improves quality of life for both patients and their caregivers. Our YMCA staff are trained instructors following the Pedaling for Parkinson’s protocol, and monitor heart rate and exercise intensity throughout each session.
Post Rehabilitation
Recovering from or preparing for a knee, hip, or shoulder replacement or surgery? Our trained specialists are ready to work with you to prepare your body before a procedure and support your recovery after a musculoskeletal injury.
What Our Programs Provide
Every Disease Management program at the Y is built around the same core commitment: meeting you where you are and supporting you every step of the way.
Evidence-Based Support
Our programs are grounded in research and follow established protocols developed for each specific condition or recovery need.
Trained, Caring Staff
Y staff leading disease management programs receive specialized training to provide safe, appropriate, and compassionate support.
Whole-Person Care
We look beyond the physical — addressing the emotional, social, and motivational dimensions of living with a chronic condition or recovering from illness.
Connection & Community
Programs like LIVESTRONG and Pedaling for Parkinson’s create a sense of belonging — a community of people who understand what you’re going through.
Coordination with Healthcare Providers
Our staff work alongside your doctors and physical therapists to ensure your program complements your medical care.
